Require driver's license number on PolicyLink deck page uploads

Description:
When a client uploads their declarations pages through the Policy Link feature on the agency's website, RiskAdvisor creates a risk profile from that submission but doesn't currently require or collect a driver's license number as part of the upload flow.

Tanner Insurance would like a way to require the client to enter a driver's license number when uploading a deck page, without making the full intake form mandatory (they don't want to force clients through the entire form just to get a license number — just this one field). Without it, the agency has to follow up by phone to get the DL# needed for underwriting.

Requested behavior: Add an optional-but-requestable required field for driver's license number in the deck-page upload flow, decoupled from the rest of the intake form.
