Client information not pulling into AgencyZoom

When a customer fills out a Quoteform on Insuribly’s website, it’s not pulling in any client information. It is only pulling in name. email and phone number.

Example Client: Renee Jarmuc. However you can take a look at any Quoteform submitted between 9/2-9/4 for this agency and it is having the same issues.
